Survey Instructions:

  1. Distribute the survey to employees. We would recommend using this survey as a template and delivering the questionnaire digitally via Google Forms

  2. Once you have collected at least 10% of the count of employee responses, you can evaluate the current state of mental health within the organization.

  3. We recommend sending the survey to the entire company, rather than just a portion of your employees, for the most accurate set of data.

  4. The more questions an individual selects the letter B for, the second option, the increased likelihood they are battling with mental health issues. 

  5. Use the survey responses to evaluate if your benefits program is offering the best level of care plans inclusive of physical and mental health.
